All of our Case Managers are tertiary competent health professionals and have the expertise and time to help you with your ongoing promote issues and health needs. We trust in consumer directed care, so a personalized package of support services is designed with you, specifically based on your identified needs. Across the 4 package levels, we give three different package types which have varying budget allocations depending on the amount of case management assistance you require.
